Subject: [PATCH] Add <fieldmap>s for the remaining CQL indexes mentioned in
 section 6.2 of the EBSCO USI specification: rec.identifier,
 net.path, dc.language, dc.format, id.fullText,
 id.peerReviewed, dc.description, dc.source, dc.publisher
 and id.seriesTitle.
